General Definition:
The PH Program Coordinator assists in the bio-psychosocial assessment, case management, and treatment of patients under the supervision of the clinical manager. Primary responsibilities include group therapy, family therapy, facilitating treatment teams, and crisis management. The PH Coordinator develops, organizes, and implements clinical and program activities; directs the implementation and ensures compliance with related agency and accreditation standards which promote optimum health care delivery. The Coordinator is responsible for participating in performance improvement activities, addressing deficits, identifying new problem areas, and developing plans of action.

•Assists clients in developing strength-based, stage-wise treatment-matched person-centered plans that are designed to address identified needs.

• Completes referral forms and clinical documentation necessary to obtain community-based services for clients as well as collaboration and communication with other SU/MH community partners.
• Assist clients with identifying changes in behavior, attitude, feelings, and thoughts that are triggers for relapse.
• Assist in the development of a relapse prevention plan and crisis plan utilizing a variety of supports including personal, community, and treatment interventions.
• Provide services and support to ensure that productivity standards are maintained at 80% for direct and indirect services to clients.
• Provides case management services for clients for support, rehabilitation, and/or crisis intervention purposes.
• Monitors and evaluates living situations, school, and/or program placements of clients to determine consistency of treatment and progress. Assists in commitment procedures when necessary as well as voluntary admissions.
• Provide education on self-help options such as 12-step programs, Dual Recovery, and other peer support groups.
• Support the use of prescribed medications to treat substance use and psychiatric disorders.
• Attends team meetings to review problems and plan solutions.
• Ensures that discharge planning activities are performed within the 72-hour requirement for clients being released from a hospital setting.
• Ensures that paperwork is maintained by agency, state, and accreditation body guidelines, policies, and procedures.
Maintains up-to-date knowledge and understanding of Medicaid Provider guidelines and other regulatory standards.
• Travels as needed to various locations throughout assigned areas to assess and evaluate the needs of clients and develop program goals.
• Maintain group attendance and meet productivity standards set by the supervisor. Work with community stakeholders to educate them on the program and maintain a flow of appropriate referrals to the program.
Primary Duties and Responsibilities
• Provides mental health evaluation, therapeutic intervention, and crisis intervention for consumers, as well as staff consultation on mental health issues.
• Provides emotional support and encouragement to consumer and their families.
• Coordinates activities with other staff as necessary, communicates psychological practice to staff members, and works with other staff members as part of the interdisciplinary treatment team.
• Maintains contacts with outside agencies and community resources to facilitate re-entry.
• Plans, prioritizes, and manages individual treatment programs by evaluating a consumer’s functioning; conducting intensive interviews; and gathering historical, legal, and technical data to determine and implement the most effective type of psychological intervention. Provides advanced psychological care to individuals and groups when this intervention is determined to be appropriate.
• Determines where specialized treatment is indicated to overcome major problems involving social dysfunctions, behavioral disorders, and/or severe risk of self or others and arranges for treatment.
• Participates in quality assurance review activities.
Maintains detailed case records and official reports on clients, charting and reviewing the treatment provided, recommendations made, progress of each case, and treatment planning, utilizing consultation as needed. Maintains statistics on caseload. Make specific referrals to psychiatrists, psychologists, medical staff, and chaplains.

Preferred Qualifications:
At a minimum the individual must meet the NC Division of MH/DD/SA requirements for a Qualified Professional.
Bachelor's degree in a behavioral science field from an accredited college/university and two years experience delivering clinical program services or a bachelor's degree in a non-behavioral science field from an accredited college/university and four years experience in delivering clinical program services. Credit may be given for previous clinical experience in other job positions, practicums, and internships as this experience relates to the provision of partial hospital service

General Position Description:
Under general supervision provides crisis support services to consumers receiving 24/7/365 crisis services. This may include the following levels of care/services; Gaylain’s House of Hope (for female staff as applicable) and/or any other service assigned by supervisor. Primary duties include supporting the medical and clinical services being provided to consumers, to include; maintaining a safe and therapeutic environment, assisting consumers with basic needs, e.g., providing bedding/towels, assisting with meal preparation, washing clothing, motivational counseling to enhance service delivery, transportation, and other duties assigned by supervisor. This position will also be responsible for transportation of consumers on a daily basis for outpatient groups at Lincoln Outpatient.

and internships as this experience relates to the provision of crisis services.
4. Preference may be given for individuals that are a Certified Peer Support
Specialist (or pursing certification) or Alcohol and Drug Counselor.

General Position Description:
Individuals in this position are considered to be licensed behavioral health professionals and at a minimum meet the requirements for NC Qualified Professional for the counselor level and full/associate level licensure for the clinician level. Individuals may provide services to consumers with substance abuse and/or mental health disorders... Individuals must maintain certification and associate (provisional) licensure in substance abuse and/or mental health.
